Does he get the irony that his personal care will be provided by the lowest paid staff within the healthcare / care system. They go unseen and unrepresented. I hope that he actually speaks to these people and dies with the full regret of knowing his stupidity.
I'm pretty sure rich people hospices would have somewhat better paid staff. Because of the implication.
Better is sadly relative. Better compared to staff in less rich people places, but not significantly.
