Skill issue
I'm dealing with the same thing. You can submit an appeal by phone, but the person who did it picked the worst fucking subject/primary reason. If you only have one or two, and not 50 smaller bills like me then you can go online and appeal it there.
They denied my appeal so now I need to contact their level 2 appeal (by fucking mail) or maybe call the state.
Im currently rolling that boulder up the hill as we speak, my friend. Best of luck in your fight against this truly kafkaesque machine.
Check with the billing/coding department of the hospital as well, they might have been the ones who fucked up.
They will invariably say that it's impossible for them to have made a mistake. It would take them 5 minutes to look it up, but the people in billing departments would rather spend 40 minutes on the phone telling you it's not possible than the 5 minutes it would take them to discover that it's exactly what happened.
Request that the billing department print a copy and send it to you for records. This is the only way I've ever seen people actually get them to check. You'll almost certainly need the information in order to successfully appeal with UHC even if the hospital did everything right.

Talk to your work's HR about this too. They may take it into consideration when it's time to sign a new contract.
as a person who is almost 40 and has had a colonoscopy every other year(and one time i had to get one 2 years in a row) since i was 26, you're gonna want that anesthesia. but i dont have united thank god, and i have a syndrome that comes with an automatic 60% chance of colon cancer so i guess its covered cause of that. i forget how many polyps ive had removed, but its probably like 6 or 7 now. One was big. Dad died a slow two year death from colon cancer. United really is the fuckin worst of the worst it seems though.
I’m so sorry for your loss. I started a new job recently and they did something kind of cool which I haven’t seen before, which is allow you to choose from a marketplace of insurers. Obviously private insurance should have never existed, but at least I’m using an insurer with a lower rate of denials rather than being forced to use the Great Satan that is United.
I appreciate that, even though it's been 25 years ago. So the scope was done under your previous employer i guess and you didnt have a choice then. And ive never had a choice of insurer at any job so that is definitely unique.
