Had something similar happen when i played for the first time, i died from reading the love letter in the ledger. A friend of mine got a heart attack in the first minute when she tried to turn off the ceiling fan.

I died to an uncomfortable chair
I thought Mr. Evrart would help me find my gun 
Mr Evrart will have some very uncomfortable questions to answer now that he has a body in his office.
I died from the combo of the heart attack trying to reach my tie in the ceiling fan and then the uncomfortable chair.
i died trying to run away from my hotel bill

I died from turning off the fan my first time as well.
The ceiling fan got a lot of us.

Heart attack after running the lovely old lady over while running out on the tab. I tried doing it with savor faire too.
My version of the game seems to spam autosave every 20 seconds and never deletes a save so maybe that’s newer behavior?
Whoever did my build shouldn't have made emotional regulation a dump stat
