This is already kinda happening in my field. Baylor currently has a program that includes your 2-year residency as part of the graduate program. Traditionally, you would graduate and then apply for residency at a hospital or clinic approved by our national governing body. The clinic or hospital would pay you way less than what you would regularly be making, but at least it is a liveable wage.

The Baylor program automatically places you with a clinic or hospital they have a deal with, where you work for free and have to continue to pay tuition for those two years. The benefit being that you don't have to actually interview or apply for a residency program.

I practice at a university hospital that declined to participate in the program. We figured that the program would soon become saturated by students with affluent parents with poor work ethic. We've dealt with residents from private universities in the past and the students tend to have little drive and have little personal skills when it comes to dealing with the underprivileged which makes up a large portion of our patient population.

So yeah.... I can see more universities adding some sort of mandatory unpaid internship onto more graduate programs as a scheme to pad out their billed credit hours. It's a win win for the university and the participating businesses, but it is horrible for the students.
