I mean I can certainly think of ways it could happen, depending on how the interface for a mostly automated navigation system would work.
Just one example, they could have a planetary coordinate system where practically any input would bring you to space near a different celestial body. Like they could be numbered from galaxy / solar system / orbit the way the postal codes work in many countries
Or even simpler, Siri misheard them
Surely they would double and triple check their heading and coordinates unless they were being pursued. But the technology could be as alien as the Infinite Improbability Drive from hitchhiker's guide.
Yeah there are just so many unknowns (and indeed, unknown unknowns) that it is kind of hard to say with any certainty what is plausible
