It should be law that all medical insurance investors to be covered by their worst policy offered, and can only pay for medical treatments through said policy.

Is you are manually investing into them, yes. Most people dont, the banks are investing for them.

No. I am describing a managed 401k.

And less than 15% of 401ks are managed. So, no, most 401ks are not what you just described.

When offered UH through my employer, I decided to pay triple and stay on my spouses insurance. They actually cover costs
