Mutuals — often shortened to "moots" — are the people who follow you while you follow them back. It sounds simple, but the mutual follow carries more social weight than a one-way connection. It's an implicit signal: I see your content, and I think it's worth my attention.

Mutuals on these types of websites (mastodon/blusky/shitter) doesn't necessarily mean friend, just that you follow them and they follow you.
