I think it is pretty obscure for just random, non-history-nerd people from places where the OG Nazi party never had a presence. I would be surprised if anyone I know in Australia would peg it as a specifically Nazi symbol (though probably at least some would recognise it as being a generically evil symbol because of its association with fictional Nazis). But Platner, a WWII nerd who got the tattoo in a country where Nazi symbols are illegal to display, was always very obviously extremely aware of what he was getting.

It is however from what I can see illegal to display Nazi symbols in Croatia, presumably including the totemkopf... So he would in fact have to have specifically asked for it.
