#13: Star Trek: The Original Series, season 1, episode 9 "Dagger of the Mind"

The Enterprise is sending and receiving supplies between the Federation penal colony on Tantalus V when a prisoner makes an attempt to escape by stowing away in a cargo container. When subdued, Captain Kirk and Doctor McCoy discover that the escapee is in fact Doctor Simon Van Gelder (Morgan Woodward), part of the colony's staff. The Administrator of the penal colony, Doctor Tristan Adams (James Gregory), explains that Doctor Van Gelder's condition was the result of an accident when he was testing experimental equipment meant to rehabilitate convicts. However Doctor McCoy becomes suspicious of Doctor Adams' story after closely examining the mentally-addled Doctor Van Gelder.
