The vet could've asked first and save the owner a lot of money.
Yeah, but, "empathy coughing" would be low on the list of my Occam's razor responses. That's like... good dog, but at the same time, why did you cost me a lot of money dog. They just exercising empathy but don't know how.
I'm sure if you brought your dog to the vet and they said "maybe it's only faking being sick to fit in" you'd say "what the fuck is wrong with you, run some tests"

Or they could've kept it to themselves and made the owner pay for more tests but they chose not to.
