I sometimes wonder if Qmail had had a clear license, if Gmail would have destroyed the self-hosted mail ecosystem so thoroughly.

It would have. Roundcube existed at roughly the same time, had a better UI, and was clearly licensed, and still went nowhere

Because delaying self-hosting email is a major pita. Maybe mostly because of Google.
