A doctor cannot be in several places at once. That's where your analogy falls apart.

You cannot advertise the fediverse to people already on the fediverse. They are already using it.
You need to advertise to people who are on things like twitch, Facebook, X etc.
This stream is not for you, thus it is twitch specific.
