Looks like a failed boot as no init is installed. The line about an I/O error is a red herring; that's what happens if the system is configured to use a floppy drive but none is actually present.

I'm not sure what you have to do to get Ubuntu to lack an init but there's always someone who finds a way (cf. people who managed to delete C:\Windows).
