Tying cherry stems in knots is a trick used to demonstrate oral flexibility, and generally taken as a sign of being good at kissing / other oral stuff. Given the context, flirty. Tying knots in your tongue is also a sign of oral flexibility, though perhaps with a tad less conscious control.
